CVA and COma diagnosis pre-screening (1 & 2 must be present)

PPS <= 40 (Allstate Hospice can determine during information visit)
1. Degree of ambulation – mainly in bed
2. Extent of disease-unable to do work
3. Self care – mainly assistance
4. Consciousness – conscious or drowsy/confused
Inability maintain hydration and caloric intake
1. Weigh loss > 10% in previous 6 months or 7.5% in 3 months
2. Serum Albumin <2.5gm/dl
3. No effective response to S/T intervention to improve dysphagia
4. Inadequate caloric/fluid intake
Comatose patients with any 3 of the following on >= 3^rdday of coma
